Fill this form to authorize us to use your credit card

I authorize My-flat.co.uk Limited to deduct the sum of £800.00 from my credit card (details given below) To secure the above booking and to be used as a deposit, I understand that this deposit is non-refundable in the event of my cancelling the booking, (Subject to the cancellation policy as detailed below). Refundable four weeks after departure. This deposit will be deducted from the card one month prior to arrival.

CANCELLATION POLICY.

IF BOOKINGS ARE CANCELLED WITHIN 7 DAYS OF THE ARRIVAL DATE THE FULL DEPOSIT HELD WILL BE NON-REFUNDABLE.
IF BOOKINGS ARE CANCELLED WITH MORE THAN 7 DAYS NOTICE OF THE ARRIVAL DATE.
£250.00 WILL BE RETAINED TO COVER ADMINISTRATION COSTS. NO REFUND IS GIVEN IF STAY IS SHORTENED.
Please note: once an extension invoice has been raised on the instructions of the client,
this cannot be altered and full monies are due.
Check in after 2pm / Check out 10 am. Monday to Friday maid service, weekly towel and linen change.
THIS IS A NO SMOKING BUILDING FROM 1ST JULY 2007. NO SMOKING IS PERMITTED IN THE APARTMENTS OR COMMUNAL AREAS.
